Naturally-occurring urinary bladder matrix
Naturally-occurring urinary bladder matrix is a dermal substitute that maintains and supports the healing environment, remodelling of site-appropriate and the functional tissue.
It is an acellular extracellular matrix graft derived from the inner lining of a porcine urinary bladder. It contains the epithelial basement membrane and collagen.
Collagens, glycoproteins, proteoglycans, mucins, elastic fibres, and growth factors and other factors make the three-dimensional network of extracellular macromolecules, called the extracellular matrix (ECM).
The use of the naturally-occurring urinary bladder matrix facilitates the repair process and remodelling the damaged tissue. It is available as a sheet or as a powder. The material goes through a complex process of decellularization, lyophilisation, disinfection, and terminal sterilization and before it is ready for use.
Application of dermal substitute is preceded by the following:
- Treatment of infection. If clinical signs are present.
- Cleaning of the wound bed, removing the slough and the exudates
- Debridement procedure, removing any necrotic tissue
- Use of the standard aseptic/sterile technique
- Hydration of the dermal substitutes using sterile saline (0.9%) or sterile lactated Ringer’s solution prior to use.
Some clinical cases may require more than one surgical steps.
Naturally-occurring urinary bladder matrix (UBM) is intended for the management of woundsincluding:
- Venous ulcers
- Diabetic ulcers
- Chronic vascular ulcers
- Tunnelled/undermined wounds
- Trauma wounds (abrasions, laceration skin tears, tissue damage)
- Partial- and full-thickness wounds
- Second-degree burns
- Draining wounds
- Surgical wounds (donor sites/grafts, post-Mohs surgery, post-laser surgery, podiatric, wound dehiscence)
There are only a few contraindications to use it:
- Patient with known sensitivity or allergy to porcine materials
- Third-degree burns
